Linking residents directly to sustainable food pantries, emergency shelter resources, rental stability programs, and transportation support.
Community Health Workshops
Interactive workshops and community sessions on health literacy, wellness, and navigating social safety net resources.
Behavioral & Mental Health Referrals
Warm hand-offs to culturally responsive counseling, substance use support, and community mental health specialists.
Family Support Services
Wraparound advocacy, youth wellness connections, and family navigation services that strengthen whole households.
Partnership & Advocacy
Working alongside local agencies and community leaders to eliminate structural healthcare disparities and build social equity.
