Addressing the primary social determinants of health by linking Fresno County residents directly to sustainable food pantries, emergency shelter resources, rental stability programs, and transportation passes.
Program Scope Includes
- Warm hand-offs to local food pantries & emergency nutrition programs
- Housing resource navigation & referral to local shelter and housing stability partners
- Public transit & non-emergency appointment ride coordination
- Utility & emergency basic needs referral support
Service Availability
Available to residents of Fresno County, including the city of Fresno and surrounding communities. Services are free or sliding-scale with no insurance requirement.
Hours of Operation
Monday – Friday: 8:00 AM – 5:00 PM
Closed on federal holidays
