About Our Services
LumiThrive Health is a 501(c)(3) Nonprofit community health navigation and social determinants of health (SDOH) case management organization serving Fresno County. We are not a direct medical clinic and do not provide emergency medical services. Our role is to help connect individuals and families with resources, referrals, and support.
